Of course there is a good chance of you getting a government medical college in UP. Scoring above 500 in NEET UG is good enough to get you a decent government medical college in India.
There are a number of good medical colleges you can choose from. Some of them are KGMU, IMS, LLRM, AMU , GSVM etc which are in UP itself.
Apart from these you may also apply for colleges in other states based on your All India Rank.
