I got my EwS certificate made , it has my father's name and not mine. The person in the department who made it said that it is made in the name of head of the family and all his dependants are covered in it. can I scan and upload this certificate?

NO , EWS certificate issued only by the name of the candidate who have applied for that certificate, If you got EWS certificate by the name of your father than you can not eligible to use that certificate. Every individual have to apply for there own EWS certificate.
Even I also got my EWS certificate and my name is Mentioned on it.
Basically for getting EWS certificate you must have to apply in SDM office or local government authority (tehsil).
Documents required for EWS Certificate are listed below :--
- Passport-sized photograph.
- Aadhaar card.
- PAN card.
- Voter ID.
- Land/property documents / Rent agreement (if living on rent)
- Affidavit/ Self Declaration.
- Income certificate.
- Residential proof
- Previous three months bank details etc.
