Hi
As you have mentioned there is a discrepancy in names in two of the documents you have submitted so it is mandatory to submit an affidavit.
An affidavit is a legal document sworn before either a notary public or a magistrate that all the details in the affidavit are true to your best knowledge.
The affidavit you will have to submit is the name correction affidavit where you will mention your correct name and a document supporting that name and the wrong or misspelled name and the document with that incorrect name which can used as proof to claim your right name if there is any document with your misspelled name, this document needs to be printed in a stamp paper with a requisite value and signed by a notary public or a magistrate.
This step will be f ollowed by the newspaper notification. Now since they have asked only for affidavit give just that.
you can even apply for it online. It may take less time that way. You can enquire with the notary office nearby as to how much time they will take to make an affidavit.
hope this clarifies!
