I have left 12 roll no in AMU entrance graduation form I will give 12 board exam in 2019. Will my form will accepted or not???

You Left the roll no. space empty. That's a really big issue in case of form filling.
Due to such an irresponsible act of yours, there is no chance that your form will be accepted.
Aligarh Muslim University is one of the oldest and prestigious universities in India. Sorry to say but a form without the roll no. is not at all considered.
If you want admission to the university you will have to resubmit the entry form that will be only possible next year or you can write them up an application regarding your problem. Maybe they can help you a bit or you will have to look for other colleges.
But for now, you must focus on your board exams and must try to score good marks in them.
