A small mistake now can be a big mess tomorrow. So , it is better to correct your mistake now only. In your case , i would suggest you to fill a new form with all the correct details carefully and the other option is to contact the exam controlling authorities , which may take some time and also not safely beneficial.
So , the best option is to fill a new form.

actually without cancelling this form there is no option to fill another form. i have open my university profile there is no option to cancel this form and fill another form for same course
