I have mentioned 12 admit card as identification identity on neet application form where surname is first and then name but according to 10th my name is first and then surname so can I take adhaar card as identification identity as it matches with the 10th certificate or can I take both also

You can take any Government ID card which can be used as an unique identity verifying card for the NEET examination. As a result, if you wish to take the Aadhar you can obviously take that, else you can take both the certificate in order to be safe.
