Dear candidate, it depends on which year you passed from Karnataka State Board. You will fill NEET code 1 if you have class 12th this year or appearing in 12th. You will fill NEET code 2 if you are a dropper(passed 12th in two-three years)

You have passed from karnataka state board i. e The Department of Pre-University Education, Karnataka which  offers Puc .

And

Code 3 :- code 3 says it is for those passed their intermediate / pre- degree examination with physics, chemistry, biology and English ,your physics, chemistry and biology should also contain practical exam.
