I have scored 212 marks in my nmat exam ,what colleges option do I have ? and do I have to fill colleges form separately in their particular website or are they already included in the exam form of nmat when it was initially filled

You have scored well in NMAT . The expected overall cutoff range for most if he top colleges is 200-220+. Some colleges will give priority to sectional cutoffs too while making the final selection , so if you have 70-75+ in all sections then, you are on a safe side for all colleges.
