You have not mentioned your category so it is difficult to just guess and say. But if I make certain assumptions then I would say incase you are from general category then honestly it is impossible for you to get government colleges and if you are really interested you might try another year or you can get into any of the private colleges. Incase you are not general and you belong to SC/ST/OBC category then you might have a chance to get government colleges but that too in later rounds because you need to score around 100 more to get decent government medical colleges.

It's quite difficult to predict that whether you will get seat or not as cut off marks for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.
But when we go through previous data then it suggests that with 381 marks you hold a very low chance for a seat in a Government Medical College
For example, in neet 2019, 539 marks for general category was the lowest mark to get a mbbs seat in Government Medical College, under Maharashtra State quota and it was for Government Medical College and Hospital, Gondia
With your marks you have chances for seat in private medical/ dental college and deemed university
