First of all congratulations on securing 502. Considering the fact that you have not stated any category, I will have to assume that you are a general category student. As you have secured 502 in NEET, I would suggest that you sit through all the rounds of NEET 2020 counselling process. Taking the worst case scenario into consideration, if not MBBS in any of the top government colleges of your respective state, you can actually get a seat in the government colleges and also the top notch private ones. Scaling it a bit down, the BDS is a stream which you have a high chance of getting. The BAMS and the BHMS are obviously in a near reach knowing your marks.

The decision is upto you but according to me you should see through the entire counselling process. Year drop is always an option that will be open to you.

If you have good chance in some of the top medical colleges, then I feel you should continue this year by taking admission in that college. If you are not satisfied and want to join medicals somehow in top medical colleges, then you can take a drop. But, if you want to pursue a course other than medical branches, you can do that also. It all depends upon your interests.
