I have to apply for Domicile certificate in Bihar. There are 3 levels, viz. Block, Sub-Division, District. Which one am I suppose to get for ICAR & NEET?

Depending on your location you can choose the option. If you are in a rural area then you can issue the certificate from the Block Development officer of your area. If you are in a Semi Urban or in a Corporation/Municipality region then you can issue it from the Sub-Divisional Officer/ Sub-divisional Magistrate. Now if you can, you can also issue it from the District Magistrate of your district. So you are open to choose from any one of the following provided your location and your ease. If its issued by any of the officers, it would be acceptable else not.

Hope you are doing well .See, fir participating in Bihar State quota counselling for NEET UG 2021 ,you need to have a domicile certificate which will show you are a permanent resident of Bihar itself .That will make you eligible for participating and getting a seat in the 85% of the seats reserved for the state . While entering your details for application of domicile certificate,which supposedly happens online ,you'll have to enter the district you belong to ,police station of your area,post office etc. Domicile issued by the SDO/BDO should be a proof that you are a permanent resident of the state and not a particular area or block there in the state .
