Before any suggestion I’d like to tell you are doing your best by trying I don’t think there is any Better substitute then trying your best :)
What I like to suggest you wish to watch English movies and read a lot of newspapers of course English newspaper this way you’ll be able to understand Various sentences which are usually used.Also when you are speaking to someone and you don’t get what they are speaking asked them to repeat it take your time don’t force yourself don’t be hard on yourself that you can’t understand or that can’t speak.

Obviously, you can remove your fear and can interact in English. As per your question, you are not able to recognize a sentence which is said to you. Thus you have the lack in verification of information through auditory compulsion. So for that you have to train your ears and your mind to recognize the auditory compulsions in English. For that you can see English Movies or News. Now you can go at youtube and start a english video or news and make the speed reduced to 0.25 or 0.5. Now see that you can understand or not. With time increase the video speed towards normal and with such a practice over a period of 2 months you will definitely see some improvements in your auditory compulsions.

Since you already know English, the problem you are facing is you're getting anxious in front of people maybe because you think if I mess up they will make fun of me. One way through which you can get past this fear is, stand in front of mirror and try speaking English. Try speaking in you friends group, among the people who you are sure won't make sun of you, if you fumble.
Secondly, what you can do, start writing. What happens is when the speed at which our brains think and the speak at which we speak are different, we stuck sometimes while speaking because we couldn't recollect. So, start writing your thoughts so that the speed of your brain thinking and you speaking will get in sync.
