I messed up last 3 years. I am from pcm stream but I literally didn't study anything in 11th and 12th. I wasted all time reading novels and watching movies. I got 76% in 12th. All colleges take entrance exam and I know nothing in my subjects. Soon I am turning 18. I'm so hopeless right now.

There is no need to be hopeless. Even if you have got average marks in 12th, it really dosen't matter, now all depends on the things you will do and path you will choose in your future. So focus on your strength, stop wasting your time, get up and change your life. You can be whatever you want.
