Dear student, you can definitely download application form from the official website of the counselling team, i.e. MCC. For this you need to login on the website, enter your application number and date of birth and download your counselling application form from there

For the payment details you should have kept a photocopy of the challan before submitting it. You can get the filled application form but the payment details may not be mentioned. If you have a reciept or challan, get a photocopy of it. If not, contact in the office for a solution or tell about your problem during counselling.They can help you with it.
