It's completely ok even if you have failed in maths but are you overall passed in your class 12th board examination with five subjects.
I am considering that you are overall passed in your 12th board examination and have just failed in maths.
Yes, you are eligible for Neet examination
This is because,
Your marks in maths is not considered for Neet eligibility so you are still eligible provided y ou must have overall passed with physics, chemistry ,biology/biotechnology and English and must have got:-

at least 50 % in aggregate of PCB i.e Physics, Chemistry and Biology/Biotechnology in 12th examination if belongs to general category
at least 40% in aggregate of PCB if belongs to st/Obc/ Sc
at least 45 % in aggregate of PCB in case of pwd candidates to be eligible.
Also,
* to be eligible for neet 2021 one has to be at least 17 years old or more by 31st December 2021
* There is no upper age limit for Neet 2021
* There's no limit for the maximum number of attempts/times one can appear in NEET.

The eligibility criteria for NEET UG entrance exam states that the candidate must have passed class 12th with minimum 50% marks in PCB and english subject combination as an aggregate from a recognised board of education.
So, even if you fail in maths and have a percentage more than 50% in PCB and english, you can apply for NEET UG entrance exam 2021.
