Make a time table that suits you , Stay consistent , commit to yourself.
Revising Three time in the last month is amazingly amazing.
Practising previous year questions , Reading NCERT textbooks of 11th and 12th standard line by line , practicing daily Mcqs, will help you secure a very good rank .
Give Grand test once a month and review your mistakes, make a notebook and add the extra points which you found new.
Study for atleast 6-7hours per day with focus and concentration .
After all this Spend some time with your family and/or go for a walk.

Go with the plan that suits you the most.
Revision is the key in last month and analysing your weak topics is most important. Try to work on your weak topics and give as many mock tests you can give. Try to go through NCERT, your notes , formulaes and revise them everyday. Previous year questions are must, preferably past 10 year papers.
As you have decided to revise your syllabus 3 times in 45 days, I would suggest you to follow this with special emphasis on NCERT and mock tests.
