Hope this was helpful!!
You have not mentioned which category you belong to . With 499, you have chances of procuring a seat in the government medical colleges when comparing the results of previous years. Last year, a minimum score of 492 was required to fetch one a seat in the U. P. Rural institute of medical sciences and research centre in general category and 429 got a seat in Lala Lajpat rai memorial medical college in obc category under state quota. But, scores and ranks vary each year with difficulty level of the question paper and availability of seats. However you can go through the following link to know about the cut off's and ranks for the previous years and calculate your chances of selection

Good luck
Here is a list of all government medical colleges in UP and their respective cut offs for admission under the State quotas for the General category in 2018. You'd require a score around this range only for a fair chance of admission here. The cut off under All India quota is also higher than this. Hence with 499 you don't seem to have much chances for admission in MBBS in a government medical college.
