Based on my own experience, I feel it will be very diffcult for you to get a medical college. The problem is that this year it's only gotten worse, as the paper was very easy as compared to previous years. So the cut off of every college is expected to be 30 to 40 marks more than the previous year cut offs.
So I would suggest that you either switch to BDS or take a drop for a year. If you work really hard, then there's nothing stopping you from getting a good college.

It would be very difficult for you to get the seat in MBBS even if you apply through the category reservation. There can be the private institutes where you can get the pais seats or managemebt quota seats. I would suggest you to go for the counselling rounds and fill the choice for the private colleges and wait for the results. Also use the NEET college predictor to have the idea about the collges that you can get with your marks.

The thing is the medical cut off this year was 490 and plus. I don't want to demotivate or anything. But it's hard to get MBBS seat for 256 marks. If you opt for MBBS so hard you can go for management seat in which the college takes lil more amount than usual to give you seat. Wishing you luck
