It depends if Provisional Certificate and Consolidated Marks Memos are already prepared by the institute and university. You can apply for corrections or change in name. You need to visit the institute and contact the examination department or the administration department and enquire about the procedure to apply for change in details in PC and CMM. You may have to pay an additional amount for it and provide valid proof which indicates your name. You can then obtain new PC and CMM with the updated details.
