According to difficulty level and overall neet score you will at least need 500 to 525 marks in neet for getting mbbs college,under general category - PWD candidate, this is for GMCs. But for taking admission in private mbbs college 400 to 450 marks are also good.

Dear applicant, if you consider the Past few years cut-off records then you will get an overview of the Telangana state admission possibilities. With 400 score and being a PWD candidate yes you do have moderate chances of admission into government colleges, but the private MBBS College chances are more. This is due to the fact that, PWD seats are very limited. But yes you can expect your admission with this score, if you don't get goverment MBBS Colleges, go for government BDS colleges.
