In aiims form ..what should we fill under qualifying exam.. ssc or inter science or other & also how to select city

i would like say qualifying exam put as 12th as i mean intermediate where they majorly consider them where as coming selection of city in case of exam center choose the one which place is nearer to travel on the day of exam
