In Medical Discontinuation bond(Wbmcc) Who is the "Witness" there whose signature is required ?Pls tell the procedure to produce such bond ?

Dear candidate,
According to the ALL India format of medical discontinuation,there is no witness required.Onky your signature and you parent signature along with you details is required on a 200 rupees stamp paper.Please check your format as according to me no such witness is required.
