in mht cet it is written to fill your name according to 10th / 12th marsheet in 10th marksheet it is NAME SURNAME in 12th marksheet it is SURNAME NAME MIDDLE NAME so what should I fill?

hope you are doing great. As per your query I would glad to tell you that they advise to write your name according to 10th marksheet to avoid spelling errors ,they dont have any intention with your arrangement . So it is good practise to write first name .midddle name and sur name . Hence, problem sorted
