Yes it is necessary to fill these forms as it is required to be with you in hard copy at the time of interview. In these you are not allowed to make any changes to your personal details but you are required to upload scanned copies of your academic record.

If you are in final year of your bachelor's then you are required to upload a certificate verifying the that the marks attained is atleast 50% for general and 45% for SC/ST.

If working then you are required to submit the evidence for the same.
