Print out of Registration Form and Choices Locked

Original Birth Certificate issued by a competent authority/ 10th Standard (High School) Board Certificate as proof of date of birth.

Original 12th standard mark sheet

Original grade/mark sheet of qualifying examination of all semesters.

Original degree/provisional certificate, if degree is completed.

Original conduct, transfer and course completion certificate from the Head of the institute last attended.
Original migration certificate from last institute attended
Original Category certificate
Original Persons with Disability Certificate (if applicable)
Three colour passport size photograph
One set of self attested photocopies of all the original documents.
