Yes, it is mandatory to get the Medical Certificate, as specified in the JoSAA official website, issued by a registered Medical Practitioner.
Kindly note that you shouldn't be using the Medical Certificate from 2019 or previous years but only use the current format.
You need to visit a doctor and get the details filled up. The most important ones being:
Make sure the doctor populate the fields without any mistake, signs and stamps it and fills the Registration Number of his license. Post this, you need to scan the document and upload a softcopy to the JoSAA website. If you aren't exactly aware of this, you may visit any cyber cafe and request them to provide you with the scanned soft copy. Try not to use Cam Scanner or similar mobile scanning devices because of improper picture quality and watermarks.

As you can see, you need to get the certificate signed by the doctor after conducting an inspection. So yes you have to satisfy all the required conditions mentioned as these will be inspected by the doctor and the result will have to given in the certificate.
