    Neet 2020 admit card is  a 3 pages PDF document.

    Self declaration form or Undertaking is attached to the admit card along with the exam center details on page 1

    It is not a separate form

    Details mentioned in NEET admit card undertaking

    1

    Declaration by candidate

    2

    COVID-19 details of an individual aspirant

    3

    Entry at the exam centre

    4

    Affixing photo, putting thumb impression and signature

    You have to download all the pages of admit card and fill all the details and carry all the required documents to the center
