West Bengal Proforma b is for candidates who are not residing in the State of West Bengal but whose parent(s) is (are) permanent resident(s) of West Bengal having their permanent home address within West Bengal. So, it will be applicable for such candidates only.
I think it will not be applicable for other states.  For any other state certificate, you should fill the domicile certificate form of that particular state.
