Kindly read the instructions mentioned in the hall ticket carefully.

If it says" to be arrested" meaning you need to get it signed by a gazetted officer

The stamp and the sign should be across the photograph and the form

If there is no mention of the word "attest", then it's not required to get it signed by anyone

A third case may arise wherein it. May require self attested photograph /document wherein you have to put your own signature
