is there any proforma with admit card in neet 2020?please help. I found no proforma with my admit card

i think proforma is not needed as the information bulletin is not updated......it the same (carbon copy) as 2019 neet information bulletin. take only what the hall ticket instructions say.

The Neet 2020 admit card comes with Important Instructions and advisory for Students regarding Covid 19. The first page is the admit card or hall ticket and the rest pages are the proforma. You have to download all the pages of the admit card. And have to paste one passport size photograph in the admit card, another one in the Self Declaration form and you have to take one additional photograph along with you which has to be pasted on the Attendance sheet. Along with the admit card, you have carry an original valid photo ID proof, PwD certificate (If applicable), transparent water bottle, personal hand sanitizer and your mouth and nose should be covered with mask properly. So where should we paste the Postcard size photo.
It is mentioned as mandatory in in the information bulletin. It is also mentioned that failure to do so will lead to disqualification of the student

You have to paste one Photograph as same as the application form in the admit card and one in the self declaration part.

But isn't the self undertaking form same as admit card where they have given space mentioning self declaration? Which self declaration form are u talking about?
