i think proforma is not needed as the information bulletin is not updated......it the same (carbon copy)  as 2019 neet information bulletin. take only what the hall ticket instructions say.

The Neet 2020 admit card comes with Important Instructions and advisory for Students regarding Covid 19. The first page is the admit card or hall ticket and the rest pages are the proforma. You have to download all the pages of the admit card. And have to paste one passport size photograph in the admit card, another one in the Self Declaration form and you have to take one additional photograph along with you which has to be pasted on the Attendance sheet. Along with the admit card, you have carry an original valid photo ID proof, PwD certificate (If applicable), transparent water bottle, personal hand sanitizer and your mouth and nose should be covered with mask properly.
