Yes medical certificate will be needed during the verification process after allotment of seat.
So you have to take out the print of the format given in annexure of josaa information and you can reach any government medical or any doctor you know and you can request to the doctor to fill after check or without check up ( whatever he / she prefer ) for the education purpose. Doctor will definitely do it .

For medical certificate, you need to visit the local doctor and ask them to make a medical certificate in the format similar to the one prescribed by JOSSA administration. Otherwise, you can also take out a printout of medical form from JOSAA business rules and ask an authorised doctor to fill it for you. Hope that helps!
