The medical fitness certificate is not required for the MBBS admissions in state quota counselling or All india counselling. If you are a person with physical disabilities then you have to prsent the certificate of disability otherwise you can simply go for the counselling rounds you can varify your documents. The list of documents required for the counselling and varification is given below.

Neet admit card and rank card or rank letter

class 10 and class 12 marksheets as the proof of birth

passport sized photographs same in the application

Seat allotment letter

Photo ID proof

Caste certificate if applicable.
You obtain a Medical Fitness Certificate, you just have to file a application for the medical fitness certificate to the superintendent or equivalent administrator in any government hospital.
They will do some regular check up of yours like Blood group test, your height and weight, eye sight, etc. It is not so hard to get a Medical Fitness Certificate.
If you have any disability, carry your disability certificate with you for admissions.
