It'll ve difficult for you to get admission into any government college under 15% All India Quota Counselling. However you can get government medical colleges under state quota counselling.

but in kerala first 2000 rank should cover with in All india rank 20000

As a general category you have less chance for 15% quota Seats but you can have the chance for 85% quota seats in your state level Gov. Colleges.
