It is going to be extremely difficult for you to get a government medical college. The number of seats for mbbs in Kerala in government medical colleges is extremely limited with respect to the number of applicants. It is so limited that it pales in comparison to the number of applicants. The total number of medical seats is only 1555. Furthermore, another disappointing aspect of it is that the number of EWS seats is only 130 of the total number of government seats. So I hope you understand the gravity of the situation and the grave situation that you are in right now which is extremely detrimental for you. To have earned a seat under the ews category, you needed to score a rank less than 3000. Even then you needed to score a good ews rank as the number of ews seats are extremely limited.

However, you have great chances of getting admission in a private medical Institute with ease. There are like 2500 private medical seats that will be filled through KEAM rank holders. You can always apply for Management Quota seats as it will be extremely helpful for you as you have pretty high rank.
