You haven't mentioned the exam or application form which you will be filling. The details in all your documents must be same. If there is any difference in the details such as you name you can either change the name in the document. In your case you may apply for new category certificate with you name as per records or you can get an affidavit stating that both the names belongs to the same person. You can fill the application form and at the time of document verification you can provide the affidavit. You need to fill the details same as given class 10th certificate unless stated the otherwise. You will. It face any problem if you make an affidavit and submit it at time of document verification.
