As per the official site of MCI, there are in total 498 Govt. Medical colleges which include Govt-society and trusts and 1930 Private colleges . There is no term as semi-government when we talk about medical colleges. Fees in Government colleges vary from state to state. Among all, government colleges have the least fee structure starting from Rs. 5000 p.a. to 1 lac p.a. In Govt-society and trusts colleges, fees may vary from 2 Lpa to 4Lpa. Only private medical colleges charge high fee starting from 15 Lpa to 50 Lpa which can cover the fee structure that you have mentioned in your questions. But as per the student reviews, it is recommended that you give preference to government affiliated medical colleges over private ones as academically, infrastructure and placement wise they provide better faculty.
