Its not always necessary to mention your name & date in your passport size photo taken for your exam hall but however it depends, as if your college/school is asking you to do that then you should mention your name & date otherwise It isn't necessary to have name & date along with your picture.

however, you must ask about this requirement with your teachers & do as guided.

also you haven't mentioned what sort of exam are you exactly talking about.

You have not mentioned for which exam you have this query related to passport size photo as different exams have different rules . That's why I am unable to solve your query . So ask your question again with the exam for which you want yo ask . But I will suggest you that you should take your passport size photo with you but do sign and all at the exam centre they will inform you about that .
