These are the subjects you can not learn without going into the concepts.
First of all you should remember the theory and formulas and where these formulas .
Chemistry contains the most theory part and you should read again and again this theory as most of question are from theoretical concepts. You should focus on physical chemistry and inorganic chemistry because you can do it with yourself easily from a good book .
In physics you should solved a good quality question and try to find out the concepts which is being used in the question and study each and every concepts deeply and thoroughly .
You should follow some good books and must join a test series .
Good books like : op tondon, rc Mukherjee, NCERT , HC VERMA , Cengage learning , DC pandey etc.

You don't hav much time left in your hands . But sure there is one thing that you can do to build your concept in physics and chemistry. start solving previous year question papers and see where you are stuck pick that topic and start building your concepts.
Then try to solve the questions again and see if you are successful in solving them or not.Because if you are not good enough in biology you can still study the whole chapter and you can memorize it.but when it comes to physics and chemistry you have to study the whole chapter theory and then solve the questions this is the pattern that you should follow for the preparation of any entrance test but now as we have left with less amount of time. So we can directly jump onto the questions and try to solve them and then check where we are lagging.
you must be knowing in which chapters you are not confident enough you can pick those chapters in physics and chemistry. Try to study them and then get back to questions. These are the two ways, you can choose any of the one that you like.
