Solution : The fourth option is the correct choice.

The steps of conversion are as follows:

  • The reporting verb " said to" is changed to "requested" to emphasise the imperative (request) nature of the sentence
  • The verb (fill) is changed to "to + infinitive form" (to fill) in the case of the imperative sentence.
  • There would be no change in the pronouns.
  • "This" is changed to "that" in the indirect speech.

Hence, the correct answer is: " She requested him to fill in that form ."
