You need to definitely submit a gap certificate if you took a gap between your 12th pass year and your jee qualification year. The reason behind submitting a gap certificate is telling the acceptable reasons for the gap in your studies.

No, it is not required to submit any gap certificate , for appearing for the second time in JoSAA. You are allowed to appear for the exam without a gap certificate.
Although, when you go for admissions, you will have to submit a gap certificate, if there is any gap in your passing year of class 12th & JEE year, telling the acceptable reasons of gap in your studies.
It is generally issued on a Rs.50 or Rs.100 stamp papers. You can visit your city’s Collectorate & contact any advocate in regard with the same. They will provide you a gap year & anti-ragging certificates recognised by a Gazetted officer by paying them a sum of Rs.100 each for the two.
