Should we take photographs of the signature and thumb impression and scan it or directly be scanned from the A4 sheet?

You can do either way , you can first sign it on a white paper and thumb impression on same. Then you can either scan it or you can take a photo of it and can minimize its size and upload. But the best way is to have it in a paper and scan with the scanner and upload it.
