It really depends on what board you are studying in. It is always said along with "hard" work, also do "smart" work. Maths is all about how much you practice different questions. The more questions you do, the more acquainted you get with the concept and thus your chances to face a completely new question from that concept increases. Further you need to solve previous year questions so that you get an idea of how the questions come. Sometimes we have to think straight and simple but due to our blind practice of hard questions we keep looking for harder ways to solve it.



Coming to physics, it is all about how much you understand concept. The numerical part is not that hard in physics but the concept is. The reasoning and analytical questions can only be tackled if you are thorough with your text book and truly understand the concept. Try teaching the syllabus to some weak student, it will bring out all the blind spots you missed during your study. This will help in building your concept and lastly you need to see questions related to application. Only then you can say your preparation is complete.



Chemistry is always the more learning part than  thinking. Here your reading and learning abilities will be tested. Chemistry fetches good marks only if you know your text book by heart. Be it organic or inorganic the one thing you have to do is to remember the facts. The physics chemistry is a bit easier than other two because it's about thinking and tackling problems. With formulae in your mind and some practice you will definitely not face any difficulty in scoring good marks in chemistry.

Thus I would say with all these tips and a bit of concentration and time management during exams, you can easily score full in PCM.
