With these marks and rank it's very difficult for you to get a seat in MBBS in any government college across India. Even in your own state, the minimum cutoff rank last year was 89309 in Government medical college, Palakkad. This cutoff was under state qouta in SC category.
However you can get admission in BDS government college in your state under state qouta. According to last year's cutoff, you can get admission in Government dental college, Kottayam.

As you know admission to government medical colleges is based on NEET marks is done under two categories-All India quota(15%) and state quota(85%).
With SC category and 398 marks, you might not get any government college through All India quota. But, you have high chances of getting the below mentioned government medical college for MBBS/BDS program.
- Government Medical College, Palakkad (sc cutoff:around 380)
You can also opt for private colleges.
