By any factor, if you are unable to qualify the exam, then you can risk dropping a year and then giving your exam again.
However, if your rank is too low to get a seat in Government medical colleges, then you have three options.
1. You can opt for private medical colleges in India.
2. You can also opt for MBBS studies abroad.
3. Dropping a year.
The decision will be at your hand in the latter case, however, I would recommend you to go for the 2nd case of MBBS abroad studies. It has its advantages- it just requires you to qualify your NEET exam with no requirement for the rank, the fees is much lower than private medical institutions in India and also you will get an exposure to an out-of-comfort-zone environment.
The third option will be risky because even after dropping a year there is a probability that you dont get medicals even in the next exam and that would amount to utter loss of an year.
