Based upon your score and previous year cutoff trends , you don't have chances to get a seat in government medical colleges but you have chance for government self financing college like pariyam dental college , kannur.
Or you have chances for private medical colleges like amrita school of medicine ,Amala institute of Medical Sciences, Amalanagar etc.

If I consider the previous year data .Then you have negligible chances of getting admission in any of the government colleges in mereka even through state quota .
The cutoff changes almost every year,so not much can be predicted .cutoff depends on the difficulty level of the paper and the number of students who appeared for the entrance exam.

As you know admission to government medical colleges is based on NEET marks is done under two categories-All India quota(15%) and state quota(85%).
With SC category and 345 marks, you might not get any government college through All India quota or state quota as the cutoff are usually around 450.
But, you can opt for private colleges through state quota.
