The authorities use the legal documents to check whether the persons mentioned in the Neet application form and taking admission are same or not. You can carry any government authorised document which contains your full name However if there is no such documents t, at the time of document verification you can carry a self attested affidavit claiming both the persons are same and it should be supported by relevant documents. It is the best possible and generally accepted solution. So the verification officer in would consider those documents and do the needful causing no issues to your candidature.
