Telangana board results have been already declared.
You need to fill neet application as per present. So, having already passed your Telangana intermediate , you are correct by choosing code 3.
You cannot use code 1 because,
code 1 is for those who are going to appear in 12th board 2021 examination with physics, chemistry, biology and English or have have already appeared but results are awaited i. e have not been declared.
But you have already appeared for Telangana examination 2021 , and your Telangana board results are out and you have passed it. So, you cannot choose code 1.
You need to choose code 3 because:-
Telangana state board i.e Telangana State Board of Intermediate Education offers intermediate as :-
code 3 says it is for those passed their intermediate / pre- degree examination with physics, chemistry, biology and English ,your physics, chemistry and biology should also contain practical exam.

Here's a brief a neet 2021 exam pattern as it has changed a little bit.
* Neet examination will be conducted for a total of 720 marks.
*Question will be from 4 subjects

*There will be 50 questions ( 35 + 15 ) in each subject . So, there will be a total of 200 questions ( 50 question x 4 = 200 questions).

Section A :- it contains 35 mcqs
-
Section B :- it contains 15 mcqs out of which any 10 mcqs is to be attempted
So, in each subject you have to attempt only 45 questions.
*Total number of question to be attempted
= 45 mcqs per subject x 4 subject
= 180 questions can be attempted.
* Duration of examination is 3 hours.
Marking scheme:-
* 4 marks will be awarded to you for every correct answer
* there is negative marking of one mark for every wrong answer
*No mark is deducted for unanswered questions
