Dear Reader,

You have to submit that caste certificate in which the university website shows .From my view both caste certificate format are approximately similar but you have to do what MMMUT what not what you want!!

For medical certificate you also have to refer the university website but if no format is given for that then you submit any format ,I recommend you to go for govt. hospital for medical certificate as it is valid all over India sometimes private hospitals certificate are not valid upto that extent.
