You haven't mentioned the name of the university in which you're seeking admission for Pharm D, the release of application form isn't same for all, it varies and so is the admission process it can be based on the merit of the qualifying examination, or done through entrance tests, you can mention the details in the comment box below, else you can visit the official website of the concerned university for the same,
